Claim Information

What To Do If You Have a Worker's Compensation Claim

See that the injured employee gets proper medical care.

If the injury is serious, call Carey, Richmond & Viking (or your insurance company) immediately.

Prepare the Employers' First Report of Injury Form and send it to the Department of Labor. You are required to submit this report within 10 days of your knowledge of an injury. It must be submitted even if you dispute the claim. (Note: If you are insured by Beacon Mutual or by Eastern Casualty, they will prepare and send the report for you.)

The following information must be provided:

  • Name of your company and your policy number
  • Injured employee's name, address, telephone number, social security number, and date of birth
  • Date, time, and place of injury
  • How the injury occurred
  • Type of injury
  • Name and address of medical facility used
  • Witness information
